SHORT TERM DISABILITY

Home Again is excited to offer Short Term Disability Insurance starting February 1, 2023.
This is the time to apply without any medical questions requesting information on previous conditions. Please watch the short video. Then open the enrollment form, put your name and select if you want to enroll or decline. We must have a form filled out for every full time staff by January 30, 2023.

 

Please either print and fill out the form manually, or fill out the PDF electronically.
