Intentionally small. Deeply personal.
We only serve 12–15 residents in our memory care communities.
When we say small, we mean it. Thrive at Home Again isn’t a large memory care setting disguised as small through the use of “pods” or “neighborhoods.” Our entire memory care setting serves just 12–15 residents, a deliberate choice that puts familiarity, connection, and personal attention at the center of daily life.
Industry-leading staff-to-resident ratios, dedicated nursing staff, and experienced caregivers supported by a trusted training program bring that commitment to life every day.
Current care staffing ratios
Waunakee
1 : 3.5
1 care team member for every 3.5 residents
Cambridge
1 : 5
1 care team member for every 5 residents
Columbus

These are current staffing levels. We adjust staffing as residents’ needs change and maintain at least one care team member for every six residents.

What Thrive means at Home Again Care built around the whole person.
Thrive is Home Again’s own approach to dementia care. It brings together a personal care plan, a team trained to understand each resident’s responses, and opportunities for connection and life enrichment. Support changes with the person, while dignity and familiar routines stay at the center.
Know your story
Staff learn each resident’s history, preferences, abilities, and responses so care feels personal from the start.
Understand the need
When behavior or needs change, the team looks closely at what a resident may be communicating and adjusts its support.
Life in Thrive Thrive meets each person where they are, each day.
Living with dementia can make ordinary routines and group activities difficult. Some days a resident may enjoy a gathering; other days they may need a quieter setting, a familiar voice, help with a daily task, or time with one person who knows them well.
At Thrive, we plan meaningful opportunities and adapt them to the person in front of us. We invest in staffing so our team has more opportunity for individual attention, patience, and connection.
We cannot promise that every day will be easy or that every resident will participate in every activity. We can promise to keep learning who each person is, respond as their needs change, and speak honestly with families about what we see.
Music & familiar sounds
Favorite songs, singing together, and familiar sounds can offer comfort and invite connection.
Creative expression
Art and hands-on experiences with room to participate in different ways.
Social connection
Time with other residents, caregivers, and family throughout the day.
Sensory experiences
Familiar scents, textures, and time outdoors can create moments of comfort and interest.
Familiar daily routines
For some residents, helping fold towels or set the table offers a familiar role and a sense of purpose.
Comfort & familiarity
Attention to personal routines and a setting that feels welcoming.
The Thrive Report A monthly window into daily life.
Each month, our team creates a Thrive Report for every resident, with photos from the past month and an update on what has been happening. Families often visit at only certain times of day. The report helps us share positive moments they may not have seen and keep them informed about what our nursing team is observing.
Training behind Thrive Prepared to know each person.
Crystal Long, our Director of Training & Compliance, guides ongoing dementia education as residents’ needs change. Home Again U and credential opportunities through UW–Oshkosh and the Alzheimer’s Association give our team more ways to keep learning.
