Melissa has set a new standard for how senior housing should become part of a community.  She actively works with communities to create a better environment for seniors both in Home Again facilities and beyond.  Melissa worked in a leadership position for a managed care organization for the 10 years prior to partnering with the Connery’s.  She has an expertise in both Medicaid and Medicare enrollment and eligibility as well as contracting for care providers to serve enrolled members.

Marty O’Connor

President / CEO

Marty is the managing member and a founding owner of Home Again Assisted Living.  Prior to starting the company Marty was a leading financial expert in commercial real estate and the banking industry.  While working closely with senior living banking clients and his wife Melissa he learned firsthand the need for high quality assisted living services in Wisconsin.  Marty actively works on the continued quality and improvement of not only the Home Again Communities but is also very active working with the State of Wisconsin leadership to improve the Assisted Living Industry as a whole.  Marty has led the Home Again team since its commencement in 2010.  Additionally, he has over 15 years of experience in the real estate and banking industry and was previously the Senior Vice President and Regional Director for the commercial real estate operations of a large multi-national bank.  Marty holds a degree in Finance, Real Estate and Urban Land Economics from the highly renowned Graaskamp Real Estate School at the University of Wisconsin-Madison.  Marty is an active member of the community, he is a founder and the President of Harvest in the Gardens Inc. a local charity that raises money annually to support local food banks and is an active member of the 100 Men of Dane County which commits to financially supports many local charities and service organizations each year.

 

As the President and CEO of Home Again Assisted Living, Marty leads the strategic direction of the company, oversees all financial facets, and future growth strategies for the company.

 

Marty is also the President and a managing principle of Realm Real Estate Development.

Brooke LaChance, NP

Director of Medical Operations

As the Medical Director, Brooke oversees all resident care and related activities for those living at the Home Again Assisted Living Communities. Brooke has over 14 years of experience in the health care industry. For the past 9 years she practiced as a Nurse Practitioner for the UW Health system in Madison, WI. While at UW Health Brooke specialized in Pulmonary, Critical Care Medicine, and Transplant Medicine. During this time she  had extensive experience working with geriatric patients. Prior to practicing as a Nurse Practitioner she spent 5 years working as a Registered Nurse in the UW Hospital Intensive Care Unit. Brooke received her bachelor’s degree in nursing from Linfield College in Portland, Oregon and her Master’s of Nursing Degree from the University of Wisconsin-Madison. Brooke lives in Deerfield, WI with her husband and three daughters.

Shyla Reigstad

Operations Manager

Shyla Reigstad is Home Again Assisted Livings Operations Manager. Shyla comes to us with a

Bachelors of Science; major in healthcare administration, minor in business administration. She

previously worked at DeForest Hometown Pharmacy for 6 years. She started with us as an intern

her senior year of college and has become a vital part of the Management Team that oversees

the organization. She has great experience with elderly interactions and customer service,

business management and project creation and implementation.

In her spare time she enjoys hunting, sand volleyball, spending time with family, especially her

nieces and nephew.

Crystal Long

Director of Training and Compliance

Crystal brings 17 years of Assisted Living industry experience to the Home Again Team and has been with the company since 2012. She has an associate’s degree in Health Care Administration and numerous Dementia certifications with the State of Wisconsin. Crystal is also certified to instruct Challenging Behaviors. She has extensive industry experience and has spent much of her career focusing on memory and specialized dementia care operations. Crystal created and implemented Home Agains’ Thrive Program which focuses on Thriving with Dementia. Crystal works with each Community, staff member and family to encompass our residents with individualistic care plans and hands on training.  During her career she has continued to progressively take on increased leadership roles. Crystal is a very active mother of five, her family enjoys fishing, camping, gardening and restoring old cars.

 

Crystal actively works with all Home Again Communities as our Director of Training and Compliance.

Jazmyn Knesuel

Assistant Medical Director

Jazmyn brings 8 years of caregiver experience and a degree in nursing to our Home Again medical team. Jazmyn has been a integral part of our Cambridge team while finishing her degree in nursing. Jazymn has been a distinctive piece of her local family Day Care business assisting with overall operations and the day to day routine.  Jazmyn will help lead our medical team collaborating with each community to ensure prompt and adequate care and treatment for all residents. Jazmyn comes to us from Deerfield where she lives with her Husband. In her free time she likes to travel, go to hot yoga, and spend time at her lake house.

Columbus Management Team

Michelle Burns

Community Director, Columbus

Michelle, a licensed CNA, brings 17 years of geriatric care experience to the Home Again Team and has been with the company since 2011. Her experience in the industry is extensive having worked with in-home care, skilled nursing facilities, and assisted living organizations.  During her career she has continued to progressively take on increased leadership rolls. Michelle is a life long resident of Columbus, WI and is a very active member of the community.

 

Michelle leads the Home Again Columbus operations team.

Ariann Morschauser, CMA

Community Medical Manager, Columbus

Ariann, a Certified Medical Assistant, draws on 10 years of clinical health care experience to lead the medical assistance program at the Columbus Home Again community.  For the 10 years prior to joining the Home Again team  Ariann was a Certified Medical Assistant in family practice and multiple specialty clinics in the Columbus and Madison area. Ariann has worked with a wide range of patients including geriatrics.  Ariann collaborates and builds relationships with residents and their primary care providers.  Ariann has a degree from Worsham College and received her CMA certification from Madison Area Technical College. Ariann lives in Columbus, WI with her husband and her three sons. She is very active in the community and is the Vice President of the Columbus Clubhouse Board and a member of the Columbus Recreation Board.

Melissa Meboe

Community Director, Cambridge

Melissa brings 13 years of experience as a manager and team leader in the public health industry. Before coming to Home Again, Melissa led the Rock County Human Services Crisis division as the Program Manager.  Melissa is a Dementia Capable Trainer and has experience with dementia crises. Melissa lives in Janesville and is a very active with her grandchildren and enjoys the outdoors and her dogs.

Brea Dahl

Assistant Community Director, Cambridge

Brea helps lead our team in Cambridge working closely with the Community Director and Medical Manager to ensure the best care for our residents. Brea brings years of CNA and community outreach experience to us. Brea has been with Home Again since 2017 and has taken on increased roles of responsibility since joining our team.  Brea has journeyed on numerous mission trips to benefit disadvantaged children around the world. Brea is originally from Deerfield but currently lives in Sun Prairie with her black lab, Mia. In her free time, you will find Brea enjoying the outdoors or curled up with a good book.

Lauren Loofboro, LPN

Community Medical Manager, Cambridge

Lauren, a Licensed Practical Nurse, draws on 15 years of clinical health care experience to lead the medical assistance program at the Cambridge Home Again community.  For the 10 years prior to joining the Home Again team Lauren was a nurse at the Cambridge Fort Healthcare clinic.  Lauren worked in a Family Practice clinic caring for a wide range of patients including geriatrics.  Lauren collaborates and builds relationships with residents and their primary care providers.  Lauren received her degree in nursing from Madison Area Technical College in 2005. Lauren lives in Milton, WI with her husband and her two sons.  During her free time she enjoys spending time with her family and cheering on the Packers and the Badgers.

Waunakee Management Team

Donna McPhee

Community Director, Waunakee

Donna has 18 years in Long Term Health Care experience. Before joining our Home Again team Donna was the Admissions Director at the Wauconda Healthcare and Rehab center in Wauconda, Ill. Donna has extensive experience with human development and aging. Donna received her associate degree in Child Development at Wright Junior College of Chicago in 1989. Donna also has her CNA from The College of Lake County in Illinois in 2005. Donna is bilingual and can speak and write in Polish fluently. She currently lives in Madison and in her free time she enjoys spoiling her grandchildren and enjoying the outdoors.

Bobbie Dickinson

Assistant Community Director, Waunakee

Bobbie has a Medical coding and Billing Specialist degree from Herzing College.  Bobbie has extensive experience in Long Term Care. Prior to joining the Home Again team Bobbie was the Lead Certified Nursing Assistant and Medical Records Clerk at a nursing home and assisted living located in Waunakee. Bobbie helps lead our Waunakee team working closely with the Community Director and Medical Manager to ensure the best care for our residents. She lives in Arlington with her Husband and 2 girls. She enjoys watching sports, being outdoors and spending time with her girls.

Theresa Hetzel, LPN

Medical Manager, Waunakee

Theresa, a Licensed Practical Nurse, draws on 27 years of Long-Term Health Care experience to lead the medical assistance program at the Waunakee Home Again community. Prior to joining the Home Again team Theresa was a floor nurse at a local Assisted Living and Nursing Community for 27 years. Theresa collaborates and builds relationships with residents and their primary care providers.  Theresa received her degree in nursing from Madison Area Technical College in 1991.Theresa lives in Deforest her Husband.  During her free time, she enjoys spending time outdoors hiking, biking, canoeing and traveling when she can.

Jerry Connery

Board Member, Partner

Jerry’s precision and vision in leading development projects comes from over 30 years of helping to shaping communities throughout south central Wisconsin.  During his diverse career in the development and construction industry Jerry has completed over 4,000 multi-family units including both senior and market rate units and has contributed to hundreds of thousands of square feet of commercial space.  In 1997 Jerry and his wife Patty started their first senior focus development in Sun Prairie, WI.

Marty is a leading financial expert in the commercial real estate industry and has been involved in over $500 million of development transactions during his career.  He has over 15 year of experience in the financial industry and was previously the Senior Vice President and Regional Director for the commercial real estate operations of a large multi-national bank.  Marty and his wife Melissa partnered with Patty and Jerry Connery in 2009 to create what is now Home Again Senior Living.

Andy Connery

Board Member, Partner

Andy has spent 7 years working in the development and construction industries. His experience includes all aspects of projects from the initial budgeting to hands-on field supervision. Andy has worked on projects of all scales, from single family homes to a major retail and multi-family. Andy also has experience partnering with WHEDA on the construction of senior housing.

Patty Connery, RN

Board Member, Partner

Patty has helped shape high standards of quality care in the assisted living industry during her 29 years as a Registered Nurse.  She has extensive experience in assisted living settings and has successfully opened and operated 5 assisted care facilities in 4 different communities in south central Wisconsin.  Prior to founding Home Again, Patty was the Director of Nursing Operations for skilled nursing facilities in the Madison area.
